Entrepreneurs Face 37% Jump in Health Insurance Costs for 2026
Poland's entrepreneurs saw their mandatory health insurance (składka zdrowotna) spike by 37% in 2026. The minimum contribution jumped from ~315 zł to 432.54 zł monthly for those on progressive and flat tax, due to a temporary wage-based discount expiring and rising minimum wage.
Starting February 1, 2026, self-employed workers operating under the general tax scale (skala podatkowa), flat tax (podatek liniowy), and card-tax regimes face a significant increase in their mandatory health insurance premiums paid to Poland's National Health Fund (NFZ — Narodowy Fundusz Zdrowia).
The Price Jump and Why It Happened
The minimum health contribution (składka zdrowotna) rose to 432.54 zł per month for entrepreneurs on progressive and flat-tax systems. This represents a 37% increase from 2025, driven by two factors: expiry of a temporary 75% wage-indexation relief introduced in 2025, and Poland's minimum wage rising to 4,806 zł gross in 2026.
Breakdown by tax form (2026 minimums):
- Skala podatkowa (progressive) & liniowy (flat 19%): 432.54 zł/month minimum
- Ryczałt (flat-rate tax): 498.35 zł–1,495.04 zł/month, depending on revenue tier
- Karta podatkowa (card tax): separate rate structure
What Changed from 2025
The assessment base for entrepreneurs on the general scale returned from 75% to 100% of the minimum wage starting February 2026. For flat-rate payers (ryczałt), the new reference became Q4 2025 average salary of 9,228.64 zł. A proposed reform to reduce health contribution rates across the board was vetoed by Poland's President in spring 2025 and did not become law.
What This Means for Self-Employed Expats
If you run a sole proprietorship (JDG) or one-person business in Poland, expect 432.54 zł–1,495.04 zł monthly deductions depending on your tax form and income level. Health contributions cannot be deducted from your PIT calculation (except partial offset on flat-tax earnings, up to 14,100 zł annually). Budget the extra ~120 zł/month or more if you were previously paying the temporary rate. Consider reviewing your tax form annually (by February 20) to minimize overall burden: flat-rate earners with high costs may benefit from switching to progressive or flat-tax regimes.
